New Patients
Before coming to your first appointment:
Please take a few minutes to print and fill the “INFORMATION/NEW FILE” and “ADMISSION QUESTIONNAIRE” forms or arrive 20 minutes prior to your appointment time to fill them on the spot.
Your first appointment will include:
- CONFIDENTIAL DOSSIER form revision and general health history.
- Consultation on your chief complaint.
- Informed consent explanations.
- Physical, orthopaedic and chiropractic exam.
- Complete Applied kinesiology (AK) evaluation.
- If indicated and with your consent, first chiropractic treatment using the AK approach.
- Plan 45 minutes and wear comfortable clothes.
First visit related expenses: 240 $
- Exam: 120 $
- Treatment: 120 $
- Subsequent visits: 120 $
