Before coming to your first appointment:

To optimize our first appointment, you must complete the “Admission Questionnaire” that was emailed to you when you booked your first visit. Please make sure to complete it as soon as possible before your appointment so that I can prepare your file.

If you did not receive it (even after checking your spam folder), text me at (514) 278-0012 and I will resend it to you.

Your first appointment will include:

  • Admission Questionnaire form revision and general health history.
  • Consultation on your chief complaint.
  • Informed consent explanations.
  • Physical, orthopaedic and chiropractic exam.
  • Complete Applied kinesiology (AK) evaluation.
  • If indicated and with your consent, first chiropractic treatment using the AK approach.
  • Plan 45 minutes and wear comfortable clothes.

First visit related expenses: 260 $

  • Exam: 130 $
  • Treatment: 130 $
  • Subsequent visits: 130 $

  • Cash, checks, credit cards or flash Interac
