The goal of Dr. Benoit Lambert’s chiropractic practice is to deliver exceptional care for non-surgical health issues through chiropractic treatment, complemented by the applied kinesiology approach.
We are committed to preserving the confidentiality of all personal information you share with us.
To comply with Law 25, it is important that we share this privacy policy, and we ask that you read it to stay informed about what personal information we collect and how we intend to use it. Law 25 is an Act passed by the Québec government that modernizes legislative provisions relating to the protection of personal information, which aims to protect personal information about an individual that directly or indirectly allows for their identification.
Following is our personal information management policy, which covers the collection, use, and processing of your personal information. The policy also contains important information about your rights regarding the collection of personal information.
It is important to note that we only use your information as prescribed in this policy, in accordance with the Law, and that some personal information is necessary to provide you with our services.
Reasons why we collect personal information
We collect personal information for various reasons to enable us to offer our chiropractic and health services. The primary services that we offer include chiropractic treatment, wellness consultations, health evaluations, and the scheduling of appointments. In order to offer these services, we are required to collect personal information during:
- The scheduling of appointments
- The registration of patients in our system
- The provision of health and wellness consultations
- The administration of chiropractic treatments
- Ongoing communication with patients regarding their care
- Billing and insurance processing
Please note that this policy applies exclusively to personal information of individuals as required by the Law.
Information collected
In compliance with the Ordre des Chiropraticiens du Québec (OCQ) standards we collect information for treatment records of patients that includes the name, birthday, home address, email, phone number, marital status, gender and profession.
Scope
This Policy applies to all types of personal information obtained by Dr. Benoit Lambert’s practice as part of its activities, whether it be information about its patients, partners, suppliers, employees, or any other persons (including visitors to the organization’s website).
Personal information is information:
- That concerns an individual and which directly or indirectly allows for their identification.
- A person’s professional or business contact information does not generally constitute personal information. For example: a person’s name, title, address, email address, or work phone number.
In all cases, we only collect information if we have a valid reason to do so.

Parties from whom we collect information
We may collect your information directly from you when you communicate or interact with us.
Provided personal information
If you provide us with personal information about other individuals (such as your family members or caregivers), you must ensure that you have duly informed them that you are providing us with their information and have obtained their consent to such information sharing.

Shared information will not be shared without your consent. If it becomes necessary to share data, prior consent will be requested and obtained in due time. Rest assured that we will not disclose any personal information unless you have consented to this or if we have received your authorization.

Collaborators
Third-party collaborators who may have access to information in the context of professional services have confidentiality obligations in place. They undertake to use this information only for the purposes mentioned in the agreement and to afford it the same level of protection as we do.

Consent obtainment
- When you provide us with your personal and sensitive information to receive our services, you are deemed to have consented to the collection, use, and disclosure of your personal information. However, if we need to use your information for a purpose other than the services offered, we will obtain your consent.
In addition, we do not obtain your consent in certain situations provided by law, such as, for example:
- To prevent, detect, or stop fraud;
- For the evaluation and improvement of protection and security measures;
- For use that is compatible with the purposes for which the information was collected;
- For the benefit of the individual concerned.

Our website uses cookies, such as:

- Third-party cookies that help us analyze how you use this website, save your preferences, and provide you with relevant content and advertisements.
Our website, including our online scheduling and contact forms, uses the following types of cookies:
- “Necessary” for basic website functions;
- “Functional” which allow certain features to be executed (sharing website content on social media platforms and collecting feedback);
- “Analytical” to understand how visitors interact with the website;
- “Performance” to understand and analyze key performance indicators of the website, which allows for a better user experience for visitors

Technological means used
We collect personal information through technological means such as online intake forms, secured email and printed intake forms.

Security measures
- Paper files are kept in a locked cabinet.
- No one other than Dr. Benoit Lambert himself has access to those files.
- Benoit Lambert’s Computer has a lock code when opened or after a period of inactivity.
Limited access
Dr. Benoit Lambert’s practice implements measures to limit access to personal information only to employees and individuals within the organization who have the authority to access it and for whom the information is necessary to perform their duties.
Retention period
We retain your information in physical or digital format for as long as necessary to achieve the purposes for which the information was collected or as long as required or permitted by law. The purpose, nature, and sensitivity of the information have been taken into account in determining retention periods.
Generally, to meet these obligations, information will be retained for the duration of your relationship with us and for an additional period of 5 years as per the OCQ regulations.
At the end of the retention period Dr. Benoit Lambert’s practice will ensure:
- That it is destroyed, or
- That it is anonymized (meaning it no longer allows for the identification of an individual in an irreversible manner, and it is no longer possible to establish a link between the individual and the personal information) for use in serious and legitimate purposes.
Storage locations
It is possible that personal information held may be communicated outside of Quebec, for example, when we use cloud service providers such as Microsoft OneDrive or email and the provider’s servers may be located outside of Quebec.
Information security
We make significant efforts to protect your personal information against loss, theft, unauthorized access, use, or disclosure, and against any other breach of information protection.
We have implemented a security program to monitor the constant evolution of information security threats. Among the measures adopted in our security program:
- Limiting access to personal information. Only duly authorized employees or agents who need to access your information in the course of their work have access to it.
- Managing passwords.
- Maintaining a record of incidents and an internal procedure.
- Ongoing monitoring and implementation of effective cybersecurity practices and software.
No online transmission or digital storage method is completely secure. Therefore, we cannot guarantee the absolute security of the data you transmit to us, and this is done at your own risk. However, we have implemented high security measures to protect your personal information against loss, alteration, or unauthorized disclosure.
